It's kind of hard to say.
If the figure is Genesis 8 Female based, then I would say it is likely a heavily modified, customized Futalicious. By heavily modified I'm meaning lots of morphing, looks like use of custom normal maps or displacement maps. Possibly even taking the base graft mesh into zbrush and really customizing the look, saving out as a morph target, then pulling it back to DS and applying the zbrush morph.
Of course all that being said it could also be a custom built genital mesh by MethodMad. I know which ever it is, which ever methods used, there is a lot of insane detail work on there.
