If I'm right the duplicate formulas error is due to having morph channels with the same name already in the figure as it loads, an example is if you have two sets by different vendors with a Breasts Large morph (This is part of why vendors started adding product or vendors initials into the dial/morph names) if the script to load the figure sees two identical or nearly identical names in the .dsf files as the figure loads it will throw the error. However it is sometimes a hard issue to catch in testing as unless the testers have the same sets that are causing the error it is easy to get by the testers as without both .dsf files causing the issue being present there is no error.

Though I won't say I'm 100% certain here as there are two different "duplicate X errors" and the causes are different and I'll admit I do have trouble remembering which is which.
